The state of Nevada requires that you submit a notarized affidavit so that we can verify your identity prior to sending you your certificate of completion. Once you complete your course, you'll be prompted to download the form. Once you've completed it and had it notarized, simply email the document to wecare@idrivesafely.com
Once we have received your notarized affidavit, you'll be able to download and print your certificate of completion from your account.
